EOS Business Health Assessment Tool: Organizational Checkup

The EOS® Organizational Checkup is one of the most valuable tools for understanding the true health of your business. It helps leadership teams step back, evaluate current operations honestly, and identify the areas that need the most attention.

Using a simple 1-to-5 scoring system, leadership teams rate the company across 20 critical statements that measure clarity, accountability, execution, communication, and operational discipline. A score of 1 represents weakness, while a score of 5 indicates strong alignment and execution.

The assessment begins with the Vision/Traction Organizer (V/TO), measuring how clearly the company’s vision is written, communicated, and shared by everyone. This includes evaluating Core Values, Core Focus™, the 5- or 10-Year Target™, target market clarity, 3 Uniques™, and whether the company has a defined Proven Process used consistently by the sales team.

The People Component reviews whether the organization has the right people in the right seats. This includes alignment with Core Values, seat clarity through the Accountability Chart, proper role ownership, and whether employees truly Get it, Want it, and have the Capacity to perform successfully.

The tool also measures leadership trust and team health. Open and honest communication, strong trust among leadership, and the ability to solve issues effectively are critical indicators of a healthy organization.

Traction is evaluated through Rocks, meeting pulse, Level 10 Meetings (L10s), IDS (Identify, Discuss, Solve), and the consistency of weekly execution. If meetings are inconsistent or priorities are unclear, the business will struggle to gain traction.

The Process Component reviews whether core processes are documented, simplified, and followed by all. Strong process discipline creates consistency, scalability, and fewer operational bottlenecks.

The Data Component focuses on Scorecards and Measurables—ensuring the company uses numbers to drive decisions instead of emotion. Every team member should have at least one measurable they are accountable for each week.

Finally, financial discipline is reviewed by evaluating whether the company maintains a budget and monitors financial performance consistently through regular review.

Once all 20 areas are scored, the total is added together to determine the overall percentage score that reflects the current state of the company. This score is not about perfection—it is about awareness. The real value comes from the discussions, priorities, and improvements that follow.

The Organizational Checkup should be reviewed regularly, especially during quarterly and annual planning sessions, to track progress and ensure the business continues moving toward greater clarity, discipline, and accountability.
